Welcome back. Since you have already completed the full screening within the past 12 months, you only need to provide a brief update to ensure we can support you properly during the retreat.

Please describe any changes in each of the following areas since you last completed the full form. If there has been no change in a category, simply type 'None.'

Registration is now open. However, registering below does not guarantee a spot on the retreat. Because this is a very close-knit retreat (14 people) and space is limited, after submitting your deposit and this update form, we will inform you regarding your retreat status ASAP.

If there is no space, you will be placed on the waiting list. If we cannot find a spot for you on this retreat, we will fully refund your deposit.

The below information will be kept private and read only by the teacher and the retreat manager. No information will be shared at any point.
